Applicability

This Privacy Notice describes the information collection and use practices of Dentegra, Inc.  on websites and other digital properties where it appears. This Notice does not describe our practices on platforms where it does not appear or where you are our employee or contractor. To understand how your protected health information (PHI) may be used and disclosed and how you can access this information, see our  HIPAA Notice of Privacy Practices. By using the site, our services, or providing your personal information to us, you consent to the use of your information in accordance with this Privacy Notice.  If you do not agree with any term in this Privacy Notice, you should not access and use the site, services or provide your personal information.
 

Information We Collect

The information we collect depends on your relationship with us (e.g., member, dentist, employer, provider, or broker, etc.). It may include any of the following:

Contact information. We collect your name, address, and email address. We may also collect your phone number.

Identification information. We may collect your enrollee ID number which may include SSN, employee ID or an alternate ID number. In most instances this will be done in accordance with our HIPAA Notice of Privacy Practices. We collect your date of birth if you create an account.

Payment information. We collect information about your payment card type and billing address if you are purchasing dental benefits online. Some payments may be covered under our  Financial privacy notice.

Enrollment information. We collect information when you enroll in our dental plans or register for other services.

Recruitment and professional information. We collect resumes, education, and employment history from job applicants. This includes information about your skills and qualifications for the position. Dentists may also choose to provide information about their education, specialties or professional affiliations. We may collect professional license numbers or tax identification numbers from dentists.

Site and Device Usage information. We collect logs and session data when you visit our site. We collect browser and operating system information. We may collect what site you came from or what site you are redirected to when you leave us. We also look at the areas of our site you visit and any actions you make. We collect your IP address. We also collect device identifiers. This might include your hardware model.
 

How We Use Your Information

We use information to communicate with you. We use information to respond to your questions. This includes questions about our site or plans and services. We also use information to communicate with you about our policies and terms or about your relationship with us.

We collect information about you from others. For example, our network of dental companies and affiliates may give us information about you. We may get your information from third parties who help administer our accounts or your employer’s benefit accounts. We may collect information from other third-party sources.

You Have Certain Choices

You can opt out of certain marketing. To stop receiving our promotional emails, follow the “unsubscribe” instructions in the promotional message you get from us. The types of Cookies we use are classified as follows:
 

  • Essential cookies. These technologies are necessary to ensure our website works properly and is available when you need it and help you login to our portals. Without them, the site wouldn’t function.

As a user of our online services (members, providers, benefits administrators and brokers/consultants), you can edit your email address, password, and your website preferences online.

  • Individual members, providers and benefits administrators: Contact your local Dentegra.
     

This Site is Not Intended For Children

Our site is meant for adults. We do not knowingly collect personally identifiable information from children under 13. If you are a parent or legal guardian and think your child under 13 has given us information, you can write to us or email us at the addresses provided at the end of this Notice. Security Measures

We take steps designed to secure personal information you provide through our services and through the site, although no method of data transfer over the Internet or electronic storage is 100% secure. We cannot guarantee your information’s absolute security and encourage you to use websites and share information with caution.

We Store Information In the United States

Information we collect is stored in the United States (“US”). The site is intended for people who are in the US. If you live outside of the US and choose to use this site you understand that it is at your own risk. If you live outside of the US, you understand and agree that we may transfer your information to the US. Our sites and businesses are subject to US laws, which may not afford the same level of protection as those in your country.
